Summer Reading Program Assignment Form


Use this form to generate your Summer Reading Assignment report.  When you have completed filling in all information, click the SUBMIT button at the bottom of the form.  You will be taken to a confirmation page.  Read through that page to make sure that everything is correct, then print the page and have your parents sign it.  You will be required to turn in the printed and signed copy of that form at the beginning of the school year.

3.  What is the most significant problem that the main character faces?  How is it resolved?  Be specific.(10 points)


4.  Describe the character in the book who most captures your interest.  What is it about him/her that is most fascinating? (10 points)


5.  What scene is most memorable from this book? Describe it an explain why it is memorable.  Include page numbers. (15points)


6.  What connection do you see between this book and your own life?  Does a character, event, or setting, for instance, remind you of someone or something in your life? Explain why or why not. (15 points)


7. What is the major point the author expresses? (Theme) Explain. (10 points)


8.  How does this book stretch your mind in new directions? What new ideas does it make you consider?  Be specific. (10 points)


An additional 10 points will be allotted to sentence structure, grammar, and spelling.  All answers should be written in complete, well-constructed sentences.  The assignment has a total value of 100 points.
